Is there a recipe for Ayhuasca that is guaranteed not to fail? I've read many attempts of people preparing the brew where something goes wrong, some even haven't got it right on the fifth attempt.

time/patience is key.
I've always done 3x3 (three 3-hour boils). Teaspoon of white vinegar per cook.
Do the DMT containing plant in one pot and the MAOI containing plant in another. Boil each separate.
Combine the boils of the DMT material in one pot and reduce to roughly a shots worth. Combine the boils of the MAOI containing material in one pot and reduce to roughly a shots worth.
Drink the MAOI shot first, then roughly 20-25 minutes later drink the DMT shot.
